What to Expect
You will receive a text or email up to 72 hours prior to your appointment for pre-visit registration. The pre-visit will allow you to complete any office documents, medical history, developmental screenings, sports forms, and payments from the comfort of your home. Completing this ahead of time will allow your provider time for review and save you time in the office. On the day of your first appointment we request you arrive a few minutes early. This time allows time to finish up any registration needed and ensures plenty of time to get acquainted with our staff and office. We look forward to meeting your family and joining with you to help your child maintain optimal health.

Patient forms (including developmental screenings) will be sent in your pre-visit registration text or email up to three days prior to your appointment. Please complete these in entirety prior to the day of your appointment. This will allow your provider time to review and prepare to discuss the results at your appointment.
Please provide the following information when you arrive for your visit:
- Insurance cards
- Complete immunization records
- Medical records from your previous pediatrician
