School/Sports Physicals

Summer is just around the corner, and it’s physical time!  Now is the time to schedule your child’s sports and school physicals.  If your child is going to participate in extracurricular activities the Kansas State High School Activities Association requires a yearly physical be on file at the school, and completed after May 1 of this year.  In addition, many summer camps require a physical within the last four months.

Each child after age 2, should have a yearly physical.  If your child is going to kindergarten next year they will need a physical exam prior to school entry.

Please bring the following:  Insurance card and immunizations records.  Physical forms for school or camp can be found on our website.
